POSITION OVERVIEW

This position is located in the Gallatin Rest Home and is responsible for assisting the MDS Team with resident assessments, interviews, and and plans of care. The position reports to the Rest Home Administrator and does not supervise personnel.Assist with the completion of the Minimum Data Set form (MDS) for nursing home resident assessment.Reviews individual resident charts for nursing, therapy, physician, and interdisciplinary team (IDT) documentation and assessments. Conducts resident interviews and resident testing (e.g., vision, hearing, etc.). Conducts staff interviews to collect and compile required information for use in assessments.Assists with completion of appropriate MDS assessment report for each resident, incorporating all information collected during interviews, chart reviews, and other data collection activities, in addition to interpreting instructions in the MDS manual.Completes audits including but not limited to:Pressure Injury Audit - weekly audit which includes looking at and measuring any pressure injuries, updating reports, entering information in EHR, family notifications, occasionally making rounds with Wound Clinic.AIMS and Behavior Quarterly Assessments - due January, April, July, and OctoberAssists with updating resident care area narrative assessments and care plans to ensure compliance with established timelines and guidelines.Reviews charts and MDS assessments, conducts staff and resident interviews, and develops preliminary care plans including problem statements, goals, and interventions. Forwards preliminary care plans to other care team members and nursing staff for input and approval.Reviews medical records for approved admissions and copies appropriate records to be included in resident/patient chart.Enters admission orders for new residents/patients. Assesses admission orders for possible contraindications, omissions, irregularities, and reports to staff for follow up.OTHER DUTIES AS ASSIGNED

Performs a variety of other duties as assigned by supervisor. This includes participating in special projects, occasionally filling in for other employees, participating in ongoing training, and a variety of other functions as needed. If the County Commission proclaims a local emergency due to an actual or threatened disaster such as an earthquake, fire, riot, flood etc., County employees may be required to provide services during the emergency and for a subsequent period of assessment and recovery.

QUALIFICATIONS

This work requires knowledge of human development, medical terminology, and Medicare and Medicaid payment systems, skill in clinical assessment procedures, and excellent communication and organizational skills. Attention to detail is required.

EDUCATION & EXPERIENCE

The required knowledge, skills, and abilities are typically acquired through a combination of education and experience as a Licensed Practical Nurse registered in the State of Montana.
