We are dedicated to enriching the lives of our residents by investing in you and your success!Aviara Healthcare Center is looking for a reliable, compassionate, and dedicated RN Unit Manager (NOC Shift) to join and grow with our team!Purpose: To provide our patrons with the highest quality of specialized care, comfort, and dignity.Duties and Responsibilities:Communicate regularly with residents and their family about Plans of Care, PT, OT and other treatment protocolsAttends and participates in meetings/stand up to facilitate communications with the teamAnswer residents' questions about their care, treatment plans, illness progression and all other issues so they feel safe and secure in our careMonitor and adjust resident's statuses based on changing needs and conditionsOrganize and prioritize daily work by assessing new, current and discharging residents needs in area(s) of responsibilityComplete documentation as requiredPerforms utilization review activities to provide resident appropriate, timely and cost effective careCoordinate care with resident, care providers, facilities financial services, and third party payersOversee all admissions and discharge activitiesCoordinate referrals both to and from our facilityAbility to relate positively, effectively, and appropriately with residents, families, staff and professional colleaguesAccurate charting and ability to complete necessary paperwork in a timely mannerAbility to work independently and exercise sound judgement in interactions with physicians, providers, payers and residents and their familiesMust be able to effectively communicate with, and promote cooperation and collaboration between individuals including residents/families/caretakers, physicians, nurse and other ancillary partnersSupervisory Requirements This position has no supervisory responsibilitiesSitting occasionallyLifting up to 50 pounds frequentlyReasonable accommodations may be made to enable individuals with disabilities to perform the essential functionsQualifications:Must have excellent time management skills to develop organized work processes in a high volume environment with rapidly changing prioritiesIntermediate computer skillsCompetence maintaining professional, respectful, honest interactions with residents/families and staff and partnersQualification Education and/or Experience Bachelor's Degree in Nursing or Social WorkLicensed (LVN or LPN) nurse acceptableKnowledge of Medicare, Medi cal and Medicaid programs and benefitsPCC Knowledge Language Skills Ability to read technical proceduresAbility to read and comprehend policy and procedure manualsAbility to effectively present information and respond to questions from managers and employeesMathematical Skills Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situationsReasoning Ability to solve practical problemsAbility to interpret a variety of instructions furnished in written, oral, diagram, or schedule formLVN/LPN Valid License or RN Valid Licensee in the state you are workingMust maintain all required continuing education/licensingMust remain in good standing with the Department of Public Health; License and Certification Division at all timesEnvironmental conditions: Inside work, normal temperatures, some noise, occasional fumes/odors, chemical exposure and potential exposure to bloodborne pathogens.Aviara Healthcare Center is an equal opportunity employer, in accordance with anti-discrimination laws.
