Position Summary:
Evaluates, plans, organizes, develops, and directs the Physical Therapy (PT) services for assigned patients/residents in accordance with current applicable laws and regulations, and as may be directed by the Director of Rehabilitation and/or Administrator, to assure that the highest degree of quality care can be maintained.
Qualifications:Current California PT licenseMinimum two (2) years' experience as a therapist in a nursing, hospital, or other related medical facilityTraining in rehabilitative and restorative therapy practicesCurrent CPR/BLS certificationsProven organizational and communication skillsAbility to remain calm and professional in all work-related situationsResponsibilities:
Plans, develops, organizes, implements, evaluates, and directs the physical therapy for assigned patients/residents in accordance with current laws and regulations that govern nursing care facilitiesMeets with administration, medical and nursing staff, as well as other related departments in planning therapy servicesMaintains treatment records, resident files, and progress notes as requiredAssesses patients' needs; identifies and initiates appropriate physical therapy interventionsObserves and evaluates treatment outcomes recommending changes as neededPerforms assessments and re-assessments as needed on each assigned patient/residentInterviews resident to determine interests and types of treatments neededParticipates in development and implementation of resident assessments (MDS) and care plans, including quarterly reviewsAssists in transporting resident to and from physical therapy room/areaDocuments patient care in accordance with regulatory, licensing, payer and accrediting requirementsEducates patient's family/nursing staff/caregivers in follow-through programsMaintains equipment and work area in a safe and clean conditionBenefits:
