Candidate CVs need to have the month & date on their work experience w/all gaps in employment accounted for.Includes some Chronic Pain ManagementChart closure within 72 hoursSchedule = 18 PPD, minimum see 18 PPDMonday-Friday 8-5A mix of peds/adults (Will consider IM only)A decision is typically made within 7-14 daysWe will also accept DOBE will be considered if truly BE (5 years or less since residency)Active DEA needed before the startThe following immunizations are required for credentialing (before start date): Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
