Home Care Coordinator RN:Coordinate and facilitate nursing activities including care planning and implementation, preparation for interdisciplinary/daily meeting, family conferences and related follow up.Adheres to all Empath Policies and Procedures, Empath Safety Policies and Procedures and OSHA safety guidelines.Assist staff in coordination of activities related to nursing activities, including the assessment and care plan review meetings.In conjunction with the Director, is responsible for maintaining compliance with all state and federal program requirements, obtaining and maintain certification as required by VA and managed care agencies, as well as any other state or federal law or rules.Assists in development and implementation of PACE operational policies, procedures, protocols and regulations for direct participant care and support services.Monitor's operations to ensure consistent compliance with established quality assurance standards.Responsible for interviewing, hiring process, disciplinary action, and annual performance appraisals.Organize a structured schedule for new employees to be introduced to all departments within the PACE program and to complete initial job competencies.Monitor employee fulfillment of job description and compliance with Empath Health, state, and federal regulations.Position Requirements:Graduate of a school of professional nursing program required. Bachelor of Science Registered Nursing (BSRN) strongly preferred.Current Florida Registered Nurses License and Cardiopulmonary Resuscitation (CPR) certification required.A minimum of five (5) years of health care experience, and three (3) years of management experience, preferably in a geriatric care setting required. A minimum of one year's experience in working with the frail or elderly population.Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.Must have a valid driver's license, proof of insurance and have means of transportation.Suncoast PACE (Program of All-Inclusive Care for the Elderly) is a nonprofit provider of comprehensive health care and support services to Pinellas County seniors with chronic health conditions.
