QUALIFICATIONS:Current licensure to practice Physical Therapy in the state you are workingPossess and maintain a valid Driver's License requiredAbility to work independently while adhering to established clinical protocols and guidelinesExcellent oral and written communication skillsExcellent organizational and time-management abilities to effectively manage caseloads/meet deadlinesPossess the physical ability to perform job related duties, which may require sitting, lifting, standing, bending, stooping, stretching, walking, pushing, and pullingA health screen or examination may be required prior to assignment and periodically thereafter, depending on the specific location policy, local or state regulations to verify physically capability to perform assigned duties with or without reasonable accommodations ESSENTIAL JOB FUNCTIONS: Provide physicaltherapy services as prescribed by thephysician that can be safely provided in the homeConduct initial evaluations, and admission visit within 48-72 hours of referral for patients being admitted for physical therapy; in addition, develop a plan of careCollaborate with a multidisciplinary team, including physicians, nurses, case managers, OT, to coordinate patient care and optimize treatment outcomesProvide overall supervision of physical therapy assistantsDeliver evidence-based physical therapy interventions, including exercises, manual techniques, and modalities, to improve patients mobility, strength, and overall functional abilitiesMaintain accurate and thorough documentation of patient progress in compliance with regulatory requirementsEducate patients and/or caregivers, families in the care and proper use of equipment and devicessuch as wheelchairs, braces, walkers, crutches, canes, and other prosthetic and orthotic devices.
