Job Title: Occupational Therapist - Home Health Job Specialty: Home Health Care Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 16:30) Guaranteed Hours: Not specified Experience: Not specified License: Valid state license required Certifications: Basic Life Support (BLS) Must-Have: - Flu vaccine required with no exemptions- Documentation systems: Pointcare/HCHB/OASIS E- Patient Age Group: Adults, Geriatrics Job Description: - Provide occupational therapy services to adult and geriatric patients.- Manage a daily patient load of 6 points per day with productivity standards of 30 points per week.- Perform comprehensive assessments, create and implement individualized care plans, and perform reassessments.- Work with conditions such as amputee rehabilitation, arthritis, CVA, dementia/Alzheimer's, hand injuries, traumatic brain injuries, lower extremity injuries, multiple sclerosis, nerve injuries, Parkinson's disease, spinal cord injuries, tendon repair, upper extremity rehabilitation.- Utilize adaptive equipment and assess home environments for safety and accessibility.- Conduct home assessments and assist with pre-discharge planning.- Evaluate and position wheelchairs, and work with hip and knee replacements.- Enforce infection prevention protocols and conduct fall/risk assessments.- Educate patients and families on safety, injury prevention, and management of conditions.- Required skills include lifting and transferring patients.*Note: Scrubs are not provided; preferred colors are navy pants and a white or navy top. Additional modules are required before the start and potentially during orientation, and can vary by specialty.*
