Physician for In-Home Geriatric CareLocation:
Northern Chicagoland AreaSchedule:
Monday - Friday, 8 AM - 5 PMOn-Call Schedule:
Once every 20 weeks (telephone only, 5 PM - 11 PM)We are seeking a compassionate and dedicated physician to provide primary care to our geriatric patient population in the comfort of their homes across the Chicagoland area. This position offers a rewarding opportunity to enhance the quality of life for seniors while working in a collaborative and supportive environment.Key Responsibilities:
In-Home and Virtual Care:
Deliver primary care services to geriatric patients, addressing a range of acute and chronic conditions, including CHF, COPD, CKD, DM, and more.Patient Management:
Oversee a panel of approximately 300 patients, seeing an average of 3-5 patients per day.Collaborative Approach:
Work within a multi-disciplinary team to provide comprehensive and coordinated care.Qualifications:
Education:
Medical degree (MD or DO) with a focus on geriatrics or primary care.Licensure:
Active medical license in the state of Illinois.Skills:
Strong communication skills and a commitment to patient-centered care.Compensation and Benefits:
Base Salary:
$250,000 per year, plus mileage reimbursement at the federal rate.Incentives:
Outcome-based quality bonus.Sign-On Bonus:
