Geriatrics Nurse Practitioner

Facility in Mobile, AL seeking Geriatric NP locums coverage Rates: $110/hr Program is seeking a Locum Nurse Practitioner to provide primary medical coverage in collaboration with PO physicians. The role is essential in delivering a full spectrum of health care services to frail elderly individuals who require nursing home care but are served in their communities whenever possible. Responsibilities:

• Provide primary medical care to PACE participants, including initial, semi-annual, and annual assessments.
• Manage acute and chronic conditions, prescribe medications, therapies, and treatments as needed.
• Participate in discharge planning and coordinate with the Medical Director and Clinic Supervisor on clinical policies and standards of care.
• Make nursing home and/or home visits as necessary.
• Advise on ways to improve service delivery and accountability.
• Participate in provider on-call rotation as required.

Qualifications:

• Master's degree in Nursing (REQUIRED)
• Minimum of Five (5) years of experience, preferably in long-term care (REQUIRED)
• Active Alabama License (REQUIRED)
• BLS, CDS, DEA Certifications (REQUIRED)
• Board Certified by ANCC or AANP (REQUIRED)
• Certification as a Nurse Practitioner with prescriptive authority (REQUIRED)
• Experience working with the frail or elderly population (Minimum of 1 year strongly preferred)
• Valid driver s license, proof of insurance, and means of transportation (REQUIRED)
