position. The RN will provide safe, high-quality in-home nursing care to patients receiving infusion therapy. This role involves comprehensive patient assessments, administering IV medications, managing infusion equipment, and ensuring patient comfort and satisfaction. The ideal candidate is patient-focused, detail-oriented, and comfortable working independently in a home setting.

0.6FTE (48 hours/2 weeks) in the South West Minnesota area to cover Home Infusion visits from Sioux Falls, SD and Marshall, MN.

As a key member of the interdisciplinary care team, the Home Health Infusion Nurse coordinates the delivery of care and services appropriate to the needs of the patient and their families. This includes physical and psychosocial assessment, identification of interventions, care plan development, education and monitoring.

This position must be able to work independently, be detail oriented and prioritize multiple tasks in a fast paced environment. Additionally, this position requires a reliable means to travel to multiple sites throughout the shift for patient visits!

Responsibilities Job Description

Deliver care as delegated by physician, providing accurate and clinically appropriate infusion therapy assessments and interventions for all therapies, access devices and disease processes; assessing patient for health status changes, support systems and home environment, updating the care team and documenting in clinical record as indicated.

Partner with the patient, family and care team in developing and implementing plan of care with specific goals under the direction of the physician. Initiate and coordinate care conferences with the participation of all members of the care team as needed.

Provide education and support to patients and caregivers that enable them to safely and optimally self-administer medications and solutions. This includes storage, aseptic technique, equipment use, side effects and potential complications.

Coordinate the discharge of patients when goals are met with the physician and other subject areas to achieve an orderly transition for the patient/family.

Facilitate and lead home infusion referrals as assigned.

Qualifications

Required

EducationGraduate of an accredited school of nursingExperience

2 years of recent nursing experience in an acute care setting, preferably in critical care, oncology or med/surgKnowledge of computer applications in order to perform job duties: electronic charting, etc.License/Certification/Registration

Current MN Nursing LicenseValid Driver's LicenseCurrent and adequate automobile insurance policyCurrent American Heart Association issued Basic Life Support (BLS)Preferred

Education

Bachelor of Science in Nursing or equivalent similar health related fieldExperience

1 year of previous home care/home infusion therapy experienceExperience with pediatrics, geriatrics and oncology patientsPrevious experience in infusion therapy or vascular access highly preferredLicense/Certification/Registration

Certification in infusion therapy, oncology or critical careOCN preferred. Wisconsin License Preferred OR willing to obtain
