Meets the patient's goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining physical therapy treatment plans in consultation with interdisciplinary team.Diagnose and manage movement dysfunction and enhance physical and functional abilities.Restore, maintain, and promote not only optimal physical function but optimal wellness and fitness and optimal quality of life as it relates to movement and health.Prevent the onset, symptoms, and progression of impairments, functional limitations, and disabilities that may result from diseases, disorders, conditions, or injuriesRecommend use of assistive devices and provide gait training, as deemed necessaryCompletes discharge planning by consulting with physicians, nurses, social workers, and other health care workers; contributing to patient care conferences.Documents patient care services via EMR charting using defensible documentation in a timely and concise manner, in compliance with industry standards.Maintains safe and clean working airbnb environment by complying with procedures, rules, and regulations.Protects patients and employees by adhering to infection-control policies and protocols.Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.Develops rehab and hospital staff by providing information; developing and conducting in-service training programs.Complies with federal, state, and local legal and certification requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.Strives to: promote health, create a safe and effective treatment environment, motivate others, have a good bedside manner, listen, analyze information, be quality focused.Participates in ongoing professional development to maintain a level of clinical practice consistent with best practice standards.Critically assesses self to improve clinical and professional performance.Advocates for the physical therapy profession.Performs other duties as assigned.Collaborate with the school-based team to help determine and carryout Individualized Education Programs (IEPs); identify a students annual goals and determine the colors, supports, modifications, and accommodations that are required for the student to achieve them.Provide in-home physical therapy services to homebound patients and work in collaboration with the home health interdisciplinary team.Schedule Information
