Perform initial and ongoing assessment of the patient; demonstrate exceptional clinical judgment and advanced assessment skills; communicate and document assessment with interdisciplinary team; utilize all available resources in order to meet the needs of the patient and familyPlanning & Implementation. Develop and document plan of care, treatment and services; collaborate interdisciplinary health team to deliver care; maintains an appropriate nurse-patient relationship and provides an exceptional experience for the patient and familyEvaluation & Education. Evaluate, communicate and document patient responses to care; revise plan of care based on ongoing evaluations; ensure the patient receives and understands education/training specific to their needs and abilities Training. Provide training for new team members on the unit; assume role of teacher for nursing students and other students shadowing on the unit; act as a resource for other team members for both clinical and non-clinical mattersEDUCATION:AS: Nursing (Required)LICENSES / CERTIFICATIONS: BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. It may be modified in the future and eligible for additional pay components.
