Job Type Full-timeDescription****To be considered for this position you MUST be a QHP as defined by OASAS and/or OMH Operating Regulations (CASAC, LMHC, LMSW).****Additionally you must demonstrate the ability to assess individuals with diverse psychiatric and/or chemical dependency diagnose and have knowledge of Monroe County mental health, substance use, and/or residential service systems.Position Summary:Evaluate and complete intakes on incoming OASAS and OMH referrals. Assess client mental health or SUD recovery needs. Provide coordination of referrals with OASAS and OMH residential programs to ensure potential clients meet criteria for treatment-based housing. Coordinates with external referral sources to ensure a successful transition for individuals seeking residential services. Understand and be able to identify safety concerns if applicableResponsibilities:Residents Appropriate for ServicesServe as a liaison to referral sources, responding to inquiries about referrals.Screen and assess referrals, evaluating individuals for admission suitability.Coordinate Admission Safety Reviews for clients meeting risk criteria.Prepare summaries of client evaluations.Communicate admission decisions to referral sources and individuals.Coordinate placement in appropriate units for accepted admissions.Occupancy Goals MetProvide outreach and information to potential referral sources about East House services.Coordinate referrals and evaluations to optimize bed utilization.Report on referral status, trends, and service utilization to agency staff and community service providers.Documentation Standards MetMaintain documentation to meet defined standards by regulators, funders, and East House policies.EEO Statement:East House encourages diversity in the workplace; we are an Equal Opportunity Employer.Minority/Female/Sexual Orientation/Gender Identity or Expression/Veteran/Disability.East House is a proud RMAPI (Rochester-Monroe Anti-Poverty Initiative) Level Up Champion for our commitment to social justice and promoting economic mobility in our community. RequirementsPreferred Skills/Abilities:Demonstrated ability to assess individuals with diverse psychiatric and/or chemical dependency diagnoses.Knowledge of Monroe County mental health, substance use, and/or residential service systems.Basic Computer Literacy and Microsoft Word skills.Experience using multiple database programs.Required Education and Experience:Bachelor's degree in a human service-related field and two years' paid human service experience or equivalent.Must be a QHP as defined by OASAS and/or OMH Operating Regulations (CASAC, LMHC, LMSW).Competencies:Flexibility, Communication, Conflict Resolution, Teamwork, Initiative, Responsibility, Focus, Motivation, Integrity, and Results-Driven.Working Conditions/Work Environment:Traditional office or household environment.Frequent travel in Monroe County and surrounding counties.Physical/Mental Demands:Job involves sitting, handling objects, working with a computer, and communicating verbally and in writing.Occasional lifting of up to 30 lbs., walking, standing, and reaching.Finger dexterity to operate a telephone and computer keyboard.Hearing and visual acuity to hear normal conversation and read documents. Salary Description $24.00-$24.50
