Patient Assessment: Evaluating patients' medical, psychological, and social needs to develop comprehensive care plans. 2. Care Coordination: Collaborating with healthcare providers, specialists, and community resources to facilitate patient care across settings. 3. Developing Care Plans: Creating and implementing individualized care plans based on patient assessments and ongoing evaluations. 4. Patient Education: Educating patients and their families about health conditions, treatment options, and available resources. 5. Monitoring Progress: Regularly reviewing patients' progress and adjusting care plans as needed to meet changing needs. 6. Resource Management: Identifying and coordinating community resources and support services to assist patients and their families. 7. Advocacy: Acting as an advocate for patients, ensuring they receive appropriate care and resources. 8. Documentation: Maintaining accurate and timely documentation of patient assessments, care plans, and interactions. 9. Compliance and Quality Assurance: Ensuring adherence to healthcare regulations, policies, and quality standards. 10. Interdisciplinary Collaboration: Participating in multidisciplinary team meetings to discuss patient care and make collaborative decisions. 11. Crisis Intervention: Responding to emergencies and helping to manage crisis situations effectively. 12. Follow-Up: Ensuring continuity of care through follow-up calls or visits to monitor patient outcomes and satisfaction.
