The Social Worker assists the with coordination of outpatient palliative services and hospice referrals, and makes outpatient referrals as needed, ie dietitian, social work, other. The Social Worker assists in the triage of new consult patients based on reason for consult. This position documents patient SW assessments, family meetings, interactions, family dynamics, and other information in the medical record. The Social Worker assists patients with completing advanced directives and/or aids in obtaining previously completed advanced directives.Social Worker-LMSW Palliative Care - Position RequirementsEducation* Master's degree in Social, psychology, sociology or related field is required.* LMSW required.* LCSW-C/LCPC licensure preferred.Experience* Experience working within Palliative care is preferredSocial Worker-LMSW Palliative Care - Schedule* Primarily day shift, Monday through Friday.* May rotate weekend hours and work overtime if needs of patients situation require.Social Worker-LMSW Palliative Care - Benefits* At TidalHealth, team members working at least 36 hours per pay period based on 12-hour shift schedules or at least 37.5 hours for non 12-hour shift schedules and part-time team members working at least 30 hours or more on weekends only are eligible for benefits.* Benefits include medical, prescription, vision, dental, flexible spending accounts, disability insurance plans, life insurance, paid time off plans, retirement plans, tuition assistance, employee assistance, and access to on-site childcare and a credit union.Salary range: $61,734.40 - $95,680.00 Commensurate with experience
