Job Title: Nurse Navigator (RN) - Breast Health/OncologyJob Overview:As a Nurse Navigator (RN) - Breast Health/Oncology, you will play a pivotal role in providing exceptional care to patients with breast health concerns and oncology needs. Working within a collaborative team model, you will be responsible for identifying and managing high-risk patient populations, coordinating their care, and ensuring the delivery of safe, age-appropriate patient care.Your expertise in oncology nursing and your ability to navigate complex patient care scenarios will be crucial in supporting patients through their treatment journey, improving their outcomes, and enhancing their overall experience.Key Responsibilities:Patient Navigation: Identify high-risk patient populations and manage their care throughout the treatment continuum. Provide personalized guidance and support to patients and their families, ensuring they understand their diagnosis, treatment options, and care plan.Care Coordination: Collaborate with multidisciplinary teams, including oncologists, surgeons, radiologists, and support staff, to coordinate patient care. Facilitate communication between patients, families, and healthcare providers to ensure seamless care transitions.Clinical Expertise: Deliver safe, age-appropriate care in accordance with best practices and clinical guidelines. Perform assessments, develop care plans, and implement interventions as needed for patients with breast health and oncology needs.Patient Education: Educate patients and their families about their conditions, treatment options, and self-care strategies. Address questions and concerns to empower patients to make informed decisions about their care.Support and Advocacy: Advocate for patients' needs and preferences, ensuring they receive the necessary resources and support throughout their care journey. Address barriers to care and provide emotional support to patients and their families.Documentation and Reporting: Maintain accurate and up-to-date documentation of patient interactions, care plans, and outcomes.
