Adecco Healthcare is searching for an RN Case Manager to become part of our client's team in Columbia, SC - 29203. This is a direct-hire, full-time, and permanent opportunity. Pay $39-$41 per hour Shift - Monday-Friday - 8:30am-5pm or 8am-4:30pm - Occasional on-call weekend may be required Qualifications Required: - Associates or Bachelor's Degree in Nursing. - 1 year of experience as an RN. - Must have either ED, ICU, Home Health, Med-Surg, or similar experience. - Ability to work with Drains, IV Pumps, Tubes, and give meds. - Must have driver's license and reliable transportation. - BLS Certification. Preferred: - RN with Associate's Degree with experience. Job Summary We are seeking a dedicated and compassionate RN Case Manager to join our hospice care team. This role is ideal for a nurse who has a genuine passion for working with hospice patients and their families. The RN Case Manager will play a vital role in providing high-quality care and support to patients in their homes or at care facilities. Key Responsibilities: - Patient Care: Provide comprehensive nursing care to hospice patients, ensuring comfort and dignity during their end-of-life journey. - Patient and Family Education: Educate patients and their families about care plans, medications, and available resources, helping them make informed decisions about their care. - Home Visits: Conduct regular visits to patients in their homes or at facilities, assessing their needs and adjusting care plans as necessary. - Team Collaboration: Attend bi-weekly staff meetings to collaborate with interdisciplinary team members, sharing insights and strategies to improve patient care. - Relationship Building: Establish trusting relationships with patients and their families, providing emotional support and encouragement throughout the hospice process.
