About the Job:The Elliot position supports the Solinsky Center for Cancer Care in Manchester and Londonderry, including Medical Oncology, Radiation Oncology and Breast Health Services, providing ad-hoc support services and support groups for patients.
What You’ll Do:Collaborates and participates with the primary nurse and the multidisciplinary team in the ongoing clinical social work assessment of each patientIdentifies relevant community resources and links them to patients as appropriateProvides supportive counseling and crisis intervention for patient/family as neededCoordinates with key personnel at all community health facilities, which treat oncology patients, to ensure patient needs are metCoordinate’s homecare referrals, acute rehab, and nursing home referrals as definedServes, as a resource/liaison to community social service agencies relative to identified patient/family needsWho You Are:Master of Science in Social Work, RequiredOne (1) to three (3) years of experience in working with Oncology patients and cancer survivors, PreferredExcellent clinical documentation, and successful usage of EMR systems, PreferredWhy You’ll Love Us:Health, dental, prescription, and vision coverage for full-time & part-time employeesShort term, long term disability, Accident insurance, & life insuranceTuition ReimbursementReferral bonusesAccrued earned time for full-time & part-time employees403b Retirement plans, with generous employer contributionsAnd more!#CRM$10,000 sign on bonus eligible#CRM
