As a symbol of compassion and commitment, you will play a key role in organizing and overseeing personalized services, guaranteeing that our clients enjoy a life of independence and dignity within the comfort of their communities.Key Responsibilities- Perform detailed assessments to evaluate the needs and preferences of our participants.- Craft and execute tailored care plans in collaboration with participants, their families, and other caregivers.- Oversee the provision of services, guaranteeing alignment with the goals and needs of our clients.- Offer continuous support and advocacy for our participants, proactively addressing concerns.- Keep meticulous records of all services and interactions, ensuring information is current and accurate.- Work in partnership with community resources and service providers to fortify the support network for our clients.- Engage in ongoing educational and professional development opportunities to remain at the forefront of care management practices.Qualifications- Bachelor’s degree in Social Work, Nursing, or a related discipline.- A minimum of 2 years experience in care management or a similar role.- Deep understanding of the unique needs and challenges confronting older adults and individuals with disabilities.- Exceptional skills in communication, organization, and problem-solving.- Self-motivated with the ability to excel independently and as a team member.- Competency with electronic health records and relevant technological tools.What we
