Description: Essential Job Functions:Write and implement Implementation Strategy Plans, outcomes, behavior strategies and goals. Update data collections.Prepare and conduct annual and semiannual review staff meetingsSchedule and conduct in-service staff meetingsMaintain contact with OSG and family guardiansInteraction duties with clients.Maintain contact with CDS, Community Day Services programsMonthly QIDP progress notes on all clientsCIRAS reports on databaseFiling of client's paperworkNew admission dutiesMaintain contact with RN for all clientsReview ABC cards from the CILA and CDSReceive Personal Plans/Discover Tools completed by the ISC agencySend QIDP progress notes and physician orders to ISC monthlySpecial Activities with clientsMaintain contact with BCBAUpdate DHS turnarounds with new ICAP scores and send to ISC agencyPlan of correction from surveysCompletion of packets requesting additional support or services to DHSVisit each CILA on time monthly. Address needs and requests from clients. Address needs of physical structure.Staff evaluationsFacilitate individual linkage and transferTraining staff on DSPHR duties, interviewing, hiring, termination and discipline.Chair various committeesInitiate and coordinate interdisciplinary process per service planAssure participation of all team membersOther duties as assignedRequirements: Basic QualificationsHigh School diploma or GED equivalent required.Bachelor's DegreeOne year of experience working with DDQMRP Training Program completeQIDP Training CompleteValid Driver's LicenseVerified Background CheckDSP CertifiedWork ExperienceKnowledge, Skills and RequirementsWorking knowledge of supervising, Excel, Microsoft Word, general ledger, accounts payable, accounts receivable and payroll required.Working knowledge of computer hardware, software programs, Medicare and Medicaid preferred.Must be able to satisfactorily complete pre-employment back screen exam for this position, drug testing, as well as an annual body mechanics screening.Data base knowledge a plus.Physical Requirements:Ability to communicate in English via phone, in writing and verbally in conversation with different levels of staff, resident families, and any outside contacts.The physical demands described are representative of those that must be met by an employee to successfully perform the essential functions of this job.
