The Direct Support Professional (DSP)/CNA engages individuals served in a variety of enrichment activities and experiences that fosters growth and independence. The DSP promotes the importance of personal care, hygiene, and overall health and wellness.LIFE Services population: individuals with intellectual/developmental disabilities and medical support needs.Brain Health Services population: individuals with traumatic and acquired brain injuries.**Reporting Relationships****Reports To** : Nurse Manager of Employment & Community Services**Direct Reports** : No direct reports**Essential Job Functions:**_(Reasonable Accommodations may be made to enable individuals with disabilities to perform these essential functions.)_+ Provides direct assistance to individuals with self-management, feeding, bathing, indicated treatment protocols, and toileting needs.+ Conducts body checks during personal care and reports findings to the Nurse Manager.+ Distributes and records medications dispensed under the direct supervision of the Nurse manager.+ Completes daily documentation in EHR to include all pertinent information specific to individual client care facilitation and program involvement.+ Serves as secondary staff for client outings and class activities as needed. Will also be responsible for independently running specific tracs throughout the day, after initial training on material is complete and observation and mentoring received.+ Responsible for assisting individuals at mealtime to ensure safety and promote a pleasant and respectful eating environment.+ Supports individuals with the use of assistive technology as identified in individual care plans.+ Attends interdisciplinary plan-of-care conferences for assigned individuals.+ Assists in maintaining work equipment and the work area, following daily department checklists and the infectious disease/safety regulations per agency policy and supervisory directives.+ Practice and promote safety in the workplace; reports all accidents according to agency procedures.+ Performs special assignments and or projects as assigned.**Required Education, Experience, Certifications, Licensure and Credentials:** _(Where appropriate, education and/or experience may be substituted)_**Minimum Required Education:** High School Diploma or GED equivalent required.**Minimum Required Experience:** Experience working with individuals with disabilities in a day service, residential, or community setting is preferred.**Required License Certification Registration** : Must be Certified in CBRF Medication Administration; current WI Nurses Aid License, without limitations, that is incompatible with duties, responsibilities, supervision, or practice setting.**Knowledge Skills Abilities:**+ Demonstrates ability and skill with written and oral communication.+ Basic computer skills to access and record information in web-based system.+ Ability to successfully work with individuals who exhibit behavioral challenges.+ Must be able to obtain within 120 days of hire and maintain Non-Violent Crisis Intervention Certification; Must be able to employ Non-Violent Crisis Intervention Techniques and holds as indicated.+ First Aid Certification must be obtained within 120 days of hire.+ Teamwork: Promotes cooperation and commitment within a team to achieve goals and deliverables.+ Safety Focus: Adheres to all workplace and trade safety laws, regulations, standards, and practices.+ Adaptability & Flexibility: Adapts to changing business needs, conditions, and work responsibilities.**Physical Requirements, Visual Acuity, and Work Conditions:****Physical Requirements** : Must be able to safely maneuver a person in a wheelchair on different terrains. Must be able to transfer a person from a wheelchair to the toilet, chair, tub, or van seat/car with use of a gait belt and/or manual or power Hoyer lift. On occasion, must be able to move, transport, position, push/pull and/or lift objects and equipment weighing up to 100pounds for a minimum of 450 feet.**Visual Acuity** : The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal. This position is responsible for the independent supervision and safety of individuals in a variety of situations and environments.
