PSYCHIATRIC MENTAL HEALTH NURSE PRACTITIONER / HYBRID REMOTE

We are seeking a dedicated Psychiatric Mental Health Nurse Practitioner to join our innovative team focused on improving senior care. Compensation:

$160,000.00 - $185,000.00 + Bonus + Equity + Signing Bonus

Essential Duties and Responsibilities:

Provide psychiatric consultations, evaluate psychiatric illnesses, and offer counseling and treatment in collaboration with PACE Physicians.

Perform comprehensive psychiatric assessments either in person or via telehealth, working closely with other healthcare professionals to develop personalized care plans.

Coordinate with physicians and the interdisciplinary team (IDT) on discharge planning from acute or long-term care settings.

Refer participants to psychiatrists as indicated in consultation with PACE Physicians.

Oversee the care of participants in nursing homes, ensuring regular visits are made based on individual needs.

Act as a consultant to the IDT, providing updates on participant changes, collaborating on care arrangements, and coordinating continuous care delivery.

Minimum Position Qualifications:

Master’s degree in nursing with a focus on Psychiatric/Mental Health from an accredited college or university.

Certified as a Psychiatric Mental Health Nurse Practitioner.

Active State License as a Psychiatric Mental Health Nurse Practitioner with no restrictions.

Valid DEA and State CDS certifications.

Valid National Provider Identifier (NPI) license.

Possession of a valid State Driver’s License with a clean DMV record and recent Vehicle Insurance.

At least two (2) years of experience in Psychiatric Mental Health, particularly with frail or elderly populations.

Preferred Qualifications:

Experience working in hybrid remote settings and adapting to flexible work environments.

Strong interdisciplinary team collaboration skills to deliver comprehensive participant care.

Proven ability to manage a significant caseload while maintaining high standards of care.

Familiarity with electronic health records and telehealth systems.

Commitment to continuous professional development and education.

Skills:

Strong clinical skills with an emphasis on psychiatric assessments and elderly care.

Ability to work effectively as part of a multidisciplinary team.

Demonstrated ability to manage time and prioritize tasks effectively.
