Jacksonville, FL 32205Under the direction and supervision of the Homecare Coordinator, the Certified Nursing Assistant (CNA) provides personal care services to PACE participants and performs related tasks. The CNA is responsible for performing duties assigned in order to increase the level of comfort, maintain personal hygiene and ensure a safe environment for the participant. As scheduled by the Home Care Coordinator, the CNA will work in participant homes or other care settings as needed to provide services identified in participant care plans such as activities of daily living (showering, dressing, etc.) or household chores. This includes assisting the participants in activities, bathing, laundry, other daily living tasks, and escort services to medical appointments. The CNA is a member of the PACE Interdisciplinary Team (IDT).Duties and Responsibilities:Assist participants with activities of daily living, provide personal care for participants, and provide light housekeeping duties as needed and directed in the PACE Center or Home Care team and the participants plan of care.Provide emotional support to participants and their families. Observe for physical and emotional changes and report to the Home Care Coordinator or Center Manager.Survey and observe any safety issues and report to Home Care Coordinator or Center Manager.May take and record patient pulse, respiration, weight, and temperature.Maintain daily cleanliness of participant rest rooms, the day center, and ensures participants are assisted in restroom activities.May provide laundry and bathing duties as requested.Participate and guide participants in day center activities and meal and snack service.Report all complaints and grievances made by the participant to the Home Care Coordinator.Treat all participants fairly, with kindness, dignity, and respect.Relay patient-related information efficiently and effectively to appropriate staff members.Maintain up to date records using EMR system so problems, plans, actions, and goals are accurately and clearly stated according to PACE Place policies, best practice, and regulatory requirements.Successfully complete the annually required compliance and in-service training.Home care CNA must be professional in appearance and verbally, being sure to always provide quality customer service to both the patient as well as their fellow colleagues.Participate in QAPI program as appropriate.Attend regular staff meetings as scheduled.Display critical thinking skills to problem solve and manage patient and family issues according to PACE Center or Home Care team, and the PACE Place policies and procedures.Inform the Home Care Coordinator or Center Manager of unusual, problematic patient/family issues. Participate in emergency disaster processes as defined by PACE PLACE policies.Demonstrate familiarity with PACE Place policies/procedures and follows state and federal regulatory requirements.Participate in Inter-Disciplinary Team (IDT) meeting, as able and available.Maintain all HIPPA and confidentiality of participants.Follow all PACE Place Policies, Procedures, and OSHA safety guidelines.Maintain the confidentiality of all company procedures, results and information about participants, clients, or families.Manage potentially infectious specimens with appropriate biohazard precautions and practices standard precautions.Maintain safe working environment.Only act within the scope of their authority to practicePerform any other duty assigned by the Home Care Coordinator or Center Manager.Qualifications and Requirements:High School Diploma or GED, preferred.Current CNA certification in Florida.Level II AHCA Background Check.Valid Florida driver's license.Active Automobile Insurance Coverage.CPR with First Aid or BLS certification requiredExperience 6 months experience providing care in patient homes, preferred.Have 1 year of experience working with a frail or elderly population OR, if the individual has less than 1 year of experience but meets all other job requirements, the candidate must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently.Skills and KnowledgeKnowledge of principles, practices, standards, and techniques of a CNA.Knowledge of common safety hazards and precautions for a safe work environment.Knowledge of medical equipment use and maintenance.Skilled in establishing and maintain effective working relationships with participants, co-workers, medical staff, and the public.Ability to communicate clearly and effectively.Ability to react calmly and effectively in emergency situations.Basic computer literacy.Medical ClearanceEmployees must have medical clearance for communicable diseases and up-to-date immunizations, before having direct participant contact.We are an equal opportunity employer.We do not discriminate on the basis of race, color, religion, marital status, age, national origin, disability, pregnancy, genetic information, gender, sexual orientation, veteran status, or any other status protected under federal, state, or local law.
