Qualifications & Requirements• ANCC, AANP or PA Certification in Family, Geriatrics, or Adult (Gerontology PC or AC)• Active Oklahoma NP or PA license or ability to obtain• 1+ years of clinical experience• Experience with EMRs, Advanced knowledge of ICD-10 diagnostic codes• Medicare Advantage knowledge of HCC model and STARs measures• Experience in gerontology, cardiology, internal medicine or endocrinology preferred• Comfortable traveling within the market to multiple centers• Willing/enthusiastic about working with Medicare-aged, medically underserved patient population
