In effort to provide complete customer and patient satisfaction, the TC nurse will accurately listen and respond appropriately to requests made by transferring physicians, hospitals and peers and provide clinical direction. The TC nurse will manage multiple phone lines and responsibilities including transfer center logistics for patients being transferred into and out of NYC H+H facilities. Job Responsibilities:Demonstrates exemplary customer service to all stakeholders/clients of NYCHHC requesting transfers via telephone and electronic requests through EPIC Transfer Center Module.Prioritize and problem solve to meet changes in patient needs with regard to admission/transfer/resource availability.Coordinate NYC H+H functions in support of emergent, urgent, routine and elective patients to a final destination facility from all potential referral sources via a single call to initiate acceptance and expedite decision making relative to potential access in support of NYC H+H facilities.Field calls from clinicians, gather clinical information for transfer requests and facilitate transfers by matching the patient's clinical needs to the facility with the bed capacity and service capacity.Act as a clinical resource to internal and external customers, departments and clinicians involved in patient transfers.Correctly and completely collect and interpret patient condition, medical eligibility and appropriateness for transfers.Anticipate potential safety concerns and identify high risk patient situations and intervene or triage as needed.Provide pre-admission/admission of screening of all incoming patients and transfers in order to attain efficient, effective quality of care for patients.Demonstrate the individual ability to immediately triage and prioritize patient medical condition and diagnoses and communicate an accurate presentation of the patient from referring facilities for accepting physicians and nurses.Performs all other duties and responsibilities assigned by Supervisor and/or designee.Minimum Qualifications1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).Special Note:For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit.
