Ambulatory Leukemia Advanced Practice Provider (Four 10-Hour Days)Join a supportive Advanced Practice Team in a role supporting the Massachusetts General Hospital Cancer Center. This practice specializes in the care of patients with leukemia. This team has a demonstrated history of providing exquisite, evidence-based patient care. This is a fantastic opportunity for an Advanced Practice Provider (NP or PA) to work in this invigorating and collaborative environment. The leukemia APP will deliver direct care to outpatients. With an interdisciplinary team approach, this provider will promote a collaborative relationship between providers, professional nurses, and administrative staff, to ensure continuity of patient care in the management of their disease and related issues as well as to develop expertise in the field. Within this role, the advanced practice provider will be responsible for a panel of patients. Other responsibilities include caring for patients throughout their continuum of care and fostering collaboration with other providers within the hospital and clinics to facilitate patient care.The leukemia APP develops, implements, and evaluates a plan of specialized health care for this oncology patient population throughout the care continuum. Promotes a professional and collaborative relationship and effort between the advanced practice team, medical staff, professional nursing staff, and administrative staff. Strives to provide and to ensure continuity of care for patients, while being efficient in use of resources. This provider serves as a role model for professional advance practice and participates in the professional development of the team through teaching, committee involvement or quality and safety initiatives to name a few. In addition to clinical skills, this person should possess strong leadership and communication skills and be able to work effectively with other members of the team.Primary Responsibilities:
The leukemia NP/PA provides direct care to this patient population in collaboration with the attending physicians and respective disease center teams. The NP/PA will be responsible for:Performs and records the patient history and physical examination and discusses findings with the collaborating team. Obtaining and coordinating laboratory results, charting of clinical findings in the EMR, orders, nursing instructions, and other required elements.Applies advanced practice knowledge to identify, prevent or solve complex and/or recurring leukemia patient care problems.Follows and manages patients (in collaboration with MD) enhancing continuity of care.Performs (oversees/assures completion of) specialized procedures particular to the select oncology population mutually agreed upon with collaborating physician.Collaborates with staff and physicians to facilitate a comprehensive plan of care and coordinate resources and write appropriate orders improving access to care, establishing goals for the delivery of care to patients, participating in clinical research, and managing communications between physicians, patients, loved ones, and staff.Appreciates complex medication interactions.Ensuring appropriate diagnostic procedures occur in a timely matter through careful coordination with other services such as radiology, facilitating various clinic appointments, communicating with patients regarding test results, and providing coverage for the team.Educating patients regarding a variety of issues associated with chemotherapy and immunotherapyIndications and interpretation of radiology, pathology, and laboratory results.Understands the complex medical environments of cancer patients including, but not limited to, those with advanced malignancy or organ dysfunction.Writes orders which are in accord with current guidelines including chemotherapy and immunotherapy.Maintains and practices in accord with practice guidelines per state and hospital regulations.Provides consultation to both nurses and physicians and serves as a resource to staff in area(s) of expertise.Serves on appropriate intra- and inter-departmental committees.Collaborates with staff and other health care providers to meet staff and patient educational needs.Participates in the development of formal and informal educational programs for nursing and support staff.Monitors patient outcomes in accordance with defined standards of patient care.Identifies system-wide factors contributing to undesirable outcomes, such as complications and over utilization of services.Supports and conducts research efforts to advance knowledge and to promote research-based practice.Evaluates own practice and program effectiveness.Identifies pertinent patient care and health delivery problems for investigation.Evaluates current research findings for clinical specialty and determines applicability to practice.Assists nursing and medical colleagues in research efforts.Encouraged to conduct research.Responsible for own professional growth and maintenance of required credentials.Maintains the standards and educational requirements specific to the position and licensure.Identifies own learning needs and goals.Takes responsibilities for leadership and participation on committees and task forces within patient care services and the hospital.Maintains and updates clinical knowledge and skills based on current health care practice/research.Is willing to obtain procedural proficiency for hematologic oncologic specific procedures including bone marrow biopsy, lumbar puncture, and skin biopsies. Training will be provided.
