Salary: $110,001 - $125,000 per year

Starting Salary: $100k-$119K per year + annual bonuses up to $5,000

Schedule: Monday to Friday, No Weekends, No Calls

Hours: Flexible, 9 am-5 pm or 7 am-3 pm

Locations: Brooklyn, Manhattan, and Queens

The Home Care RN Case Manager sees patients in their homes.

What you will be doing as a Registered Nurse:

• Provide case management services for homecare patients
• Complete initial assessments, OASIS, recertifications
• Creates and implements a plan of care
• Collaborates with the interdisciplinary team and coordinates patient care
• Educates patients and families

Qualifications:

• Must have an active RN (Registered Nurse) license in the State of New York.
