TravelJob DescriptionAs a case management nurse (RN CM), you will collaborate with the healthcare team to assess, plan, facilitate and coordinate care for individual patients. Working within and outside of facilities, you will oversee the effective utilization of services and resources. In order to extend this high level of care, you will communicate with patients, families, other healthcare professionals, and social workers to promote patient safety, quality of care and cost-effective outcomes, for patients that require long-term care or home care after hospitalization.Minimum Requirements
