Job Title: Registered Nurse (RN) Job Specialty: Long-Term Care (LTC) / Skilled Nursing Facility (SNF) Job Duration: 13 weeks Shift: 12-Hour Nights Guaranteed Hours: 36 hours per week Experience: Minimum of 2 years License: Active Registered Nurse License required Certifications: Basic Life Support (BLS) Must-Have: - Familiarity with EPIC charting system Job Description: - Provide patient care in a post-acute care setting- Assess patient health problems and needs, develop and implement nursing care plans- Administer nursing care to ill, injured, convalescent, or disabled patients- Collaborate with a team to ensure the best quality of care- Maintain accurate, detailed reports and records- Monitor, record, and report symptoms or changes in patients' conditions- Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans- Modify patient treatment plans as indicated by patients' responses and conditions- Prepare and assist patients during examinations and treatments- Support patients and families in understanding patient conditions, care, and treatment plans
