Join our compassionate team dedicated to providing exceptional palliative care services. As an APC specializing in palliative care, you will manage a patient panel and provide consults to support individuals with terminal illnesses. This role offers the opportunity to deliver high-quality, patient-focused care in a supportive environment.
Practice Support:
- Certified Medical Assistants (CMA's), Licensed Practical Nurses (LPN's), Registered Nurses (RN's)
- Practice Manager
- Other Ancillary Services: Licensed Clinical Social Worker (LCSW), On-site Laboratory and Radiology Services, Certified Diabetes Educator (CDE), Outpatient Surgical Services
- 23-bed Inpatient Unit on the same campus
Schedule:
- Monday to Friday, 8:00am - 4:30pm
Qualifications:
- Current APC licensure in Ohio
- Board certification in Palliative Care preferred
- Strong commitment to patient care and interdisciplinary collaboration
