REGISTERED NURSE
TITLE OF IMMEDIATE SUPERVISOR: ADMINISTRATOR
RISK OF EXPOSURE TO BLOODBORNE PATHOGENS – HIGH
DUTIES OF POSITION
Maintains the quality of client care/services in accordance with the Agency’s objectives and policies, through planning, coordination, and implementation and evaluation of the home services provided
POSITION RESPONSIBILITIES
Reviews and investigates the clinical content, diagnoses, medical history, medications, fee status, and other procedures from the records of clients
Directs staff toward the appropriate use of community resources
Appraises staff performance during their probationary period and annually
Identifies and evaluates client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff
Develop the Plan of Care/Service after assessing the client and consulting the client and/or family member/responsible person
Review and update Care/Service Plans per policy
Participates in community-related functions as necessary
Secure physician orders at least every three months for continued care unless there is a pre-ordered period of no more than a year (for Medicaid clients) as applicable
Sees clients in the home setting as needed
Participates in client record reviews
Reviews each schedule for discharges and direct care staff supervisory visits
Ensures that all pertinent paperwork is in the client record and is complete.
Performs other duties as requested by the Administrator
JOB CONDITIONS
The position is per diem.
Primarily at the client’s home or place of stay and occasional lifting of medical supplies and charts.
On occasion, one may be required to bend, stoop, reach and move client weight up to 250 pounds lift and/or carry up to 30 pounds.
Travel is required to and from the client's home when an assessment of care/services is scheduled.
The ability to communicate well, both verbally and in writing is required.
The ability to access clients’ homes that may not be routinely wheelchair accessible is required.
Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care/services.
Must have a smartphone.
EQUIPMENT OPERATION
Use of multi-line telephone, calculator, and medical nursing equipment, i.e., glucometer, etc.
Use of a smartphone and caregiver app.
COMPANY INFORMATION
Has access to all client records which may be discussed with the Administrator, the Executive Director, the Director of Finance, and the Quality Assurance Department.
QUALIFICATIONS
Must be a graduate of an accredited School of Nursing
Must be a Registered Nurse licensed in Florida
Training/experience in the provision of Personal Care Services
