Summary

: Provides nutritional assessment and management of transplant candidates, recipients and living donors throughout all phases of the transplant process. Serves as a nutrition resource to medical and nursing staff and other health care team members maintaining professional competency and skills required for practice.

ESSENTIAL FUNCTIONS

:Assesses the nutrition needs of transplant patients and living donors and determines nutrient requirements based on age and disease specific needs of the patient.Develops, implements and monitors the effectiveness and outcomes of nutrition intervention in conjunction with the patient's healthcare team.Provides nutrition education to transplant patients and living donors.Participates in multidisciplinary meetings and programs' performance improvement initiatives.Interacts with co-workers, visitors, and other staff consistent with the core values of the Hospital.Qualifications

B.S. in Nutrition or Dietetics is

required3-5 years hospital/critical care experience

requiredMaster's degree in related area is

preferredExperience with solid organ transplant patients

preferredCERTIFICATES, LICENSES, AND REGISTRATION:

Registration with the Commission on Dietetic Registration (CDR) as RD or RDNMember of the American Dietetics AssociationPennsylvania Dietitian/Nutritionist License (LDN)Certified Clinical Transplant Dietitian (CCTD) certification within 2 years of becoming eligible based on eligibility criteria.
