Job Title: Hospice RN Job Specialty: Clinical-Nursing Job Duration: 9 months Shift: M-F 8a-5p | 5x8 Hr Guaranteed Hours: 40 Experience: Minimum of one (1) year experience as an RN in acute care hospital, oncology, long-term care, hospice, or home health. License: Current license to practice nursing in the state of employment. Certifications: - BLS (American Heart Association)- Current automobile insurance and valid driver's license. Must-Have: - Knowledge of hospice care and terminally ill patient needs.- Excellent patient assessment skills.- Good oral and written communication skills.- Documentation and program management skills.- Familiarity with managed care principles and regulatory guidelines.- Ability to operate nursing assessment tools and basic office equipment.- Reliable transportation for work purposes. Job Description: - Provide direct patient care services to hospice patients and supportive care to their families.- Manage an interdisciplinary group to enhance the quality of hospice care.- Perform initial and ongoing assessment of patients.- Develop and implement the Hospice Plan of Care as prescribed by a physician.- Work closely with interdisciplinary group to provide comprehensive physical, psychosocial, educational, emotional, and spiritual services.- Report any changes in patient conditions to attending and hospice physicians.- Maintain accurate and updated medical records, including interdisciplinary notes and assessments.- Practice infection control measures and standard precautions.- Monitor staff scheduling and participate in quality assessment and performance improvement programs.- Consistently promote core values and attend staff meetings.- Maintain knowledge of pain and symptom management in the dying process.- Conduct routine supervisory visits and evaluations of team members.- Provide education to patients, families, and caregivers in accordance with the Plan of Care. - Ensure compliance with policies, procedures, and regulatory requirements.- Act as a liaison between the physician, patient/family, and hospice team to stay informed of clinical issues and staffing concerns.- May make death pronouncements in the absence of an attending physician.- Supervise aides and complete competency evaluations and supervisory visits.
