Gateway Post Acute-RN JOB DESCRIPTION: RNThe NURSE Supervisor assesses resident care and manages facility employees in the provision of care and services according to professional standards of nursing practice, consistent with facility philosophy of care and county, state, and federal laws and regulations, as applicable, under the direction of the Director of Nursing. Assists in the development and implements policy and procedures consistent with current law.REPORTING: This position is responsible to the Director of Nursing.QUALIFICATIONS/REQUIREMENTS: Education: Graduate of an accredited school of nursing License: Current, active license as a Registered Nurse Work Experience: 1 year experience in long-term geriatric nursing care experience and/or training preferred Language Skills:Must be able to read, analyze, and interpret common scientific and technical information, and to be easily understood through verbal communication in the English language. Mathematical Skills:Must be able to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.Ability to perform these operations using units of weight measurement, and volume. Communication Skills:Must have exceptional communication and customer service skills, and be empathetic.Ability to effectively communicate with patients, families, responsible parties, staff, and outside resources and agencies. ESSENTIAL JOB FUNCTIONS:Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, and policies and procedures, etc., which are necessary for providing quality care.Direct the day-to-day functions of the nursing personnel.Ensure that all nursing personnel assigned to you comply with the written policies and procedures.Ensure that the nursing procedures manual is current at all times and reflects day to day nursing care procedures performed in this facility.Keys information into computer systems, including EMRParticipate in facility surveys by authorized government agencies as may be requested. 2Ensure that all nursing service personnel are in compliance with their respective job descriptions.Meet with nursing staff as well as support personnel in planning the department services programs and activities.Ensure that a sufficient number of nursing personnel are available to meet the daily needs of each resident.Ensure that all nursing personnel are performing their work assignments with acceptable nursing standards.Perform administrative duties as necessary.Make rounds with physician as needed.Ensure physicians record and sign progress orders on a timely basis in accordance with current regulations.Fill out and complete accident/incident reports.Ensure that routine documentation is in accordance with established policy and procedures.Make daily rounds of your unit during your shift.Monitor medication pass and treatment schedules.Review nurse's notes to ensure that they reflect the resident's response to care provided in accordance with the resident needs and wishes.Make periodic checks to ensure that prescribed treatments are being properly administered to evaluate the resident's physical and emotional status.Participate in the preliminary and comprehensive assessment of the nursing needs of the resident and develop a written care plan. Ensure that established post- mortem procedures are followed.Administer professional services in accordance with established policies and procedures.Review and ensure that the resident's care plan in being followed when administering care.Report all allegations of resident abuse and/or misappropriation of resident property. * Comply with resident rights ensuring that all residents are treated fairly, with kindness, dignity, and respect.Must function as a team member and have flexibility.Must be able to read, write, speak, and understand the English language.Must possess the ability to make independent decisions when circumstances warrant such action.Possess the ability to deal tactfully and harmoniously with personnel, residents and all visitors.Shows dependability by being punctual; maintains consistent attendance JOB FUNCTIONS: Cooperates with other departments when coordinating nursing services. Ensure that an adequate supply of floor stock medications, supplies, and equipment is available. Must be able to key information into computer systems to participate in employee performance evaluations, determine your shift's staffing requirements, and in making recommendations to the DON. Inform nursing personnel of new admissions. Provide direct nursing care as necessary. Ensure that department personnel, residents, and visitors follow the department's established policies and procedures. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures. If qualified, start IVs, and obtain specimens for lab tests as necessary. Make rounds with physicians. Inspect the work area daily to ensure that they are maintained in a clean and safe manner. Encourage the resident to participate in the development and review of his/her plan of care. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the facility. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices. Must not pose a direct threat to the health or safety of other individuals in the workplace. ! Attends various committee meetings as required or requested by Administrator or DON. Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the residents' needs. Assist in educational training programs. Attend and participate in mandated state, OSHA, and CDC in-service training programs. Is subject to frequent interruptions. Works beyond normal working hours, weekends, and holidays when necessary. Is subject to call-back during emergency conditions and other times as needed. May be necessary to assist in the evacuation of residents during emergency situations! Other duties as assigned by the Director of Nursing PHYSICAL CAPACITIES: (With or Without the Aid of Mechanical Devices)! Must be able to move intermittently throughout the workday.Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met. ! Must meet the general health requirements set forth by this facility's policies, including an annual TB screening and physical examination. Must be able to stand for extended periods of time. Must be able to lift and carry up to 25 pounds. ! Requires infrequent lifting of weights up to 100 pounds with assistance. Frequent pushing and pulling of carts! Must be able to cope with the mental and emotional stress of the position. ENVIRONMENTAL CONDITIONS: Inside work, normal temperatures, some noise, occasional fumes/odors, chemical exposure, and potential exposure to blood-borne pathogens. This job cannot be performed without exposure to the stresses associated with an intimate, 24-hour skilled care environment that delivers care and services primarily to disabled and cognitively impaired residents in an aging population. Examples of these stresses include but are not limited to: emergency health or safety response, weekend and holiday duty, unusual or impaired behavior by residents, family reactions to having a loved one in a nursing home, death and dying, oversight of state surveyors, ombudsmen and federal officials, presence of consultants and attorneys, and variable involvement of medical staff. HIPAA is a law that must be followed by all healthcare personnel at every level. This position is a Mandated Reporter regarding the suspicion of elder abuse and may have access to Protected Health Information (PHI).
