Provides educational and clinical services in a clinical setting on disease or disorder self-management following clinical standards of care. Maintains up-to-date knowledge on disease or disorder related issues in order to serve as a resource to the patient community and participate in events designed to increase health awareness. Performs patient care activities for patients of diverse ages in a clinical setting. Advocates and utilizes the nursing process in the care of all age populations along the health continuum.

Minimum Education
Graduate of an accredited school of nursing, BSN preferred or required as per specialty credentialing requirements.
Minimum Work Experience
2 year clinical experience in chronic disease care in specialty area being sought.
Experience in educating a diverse population on healthcare issues is highly desirable.
Required Licenses/Certifications
Licensed in the State of Vermont.
BLS Certification through American Heart Association.
Appropriate credentialing per specific discipline or to be acquired within 1 year of obtaining the position.
Required Skills, Knowledge, and Abilities
Basic Microsoft Windows desktop application and navigation skills.
Excellent verbal and written communication skills.
Demonstrated organizational and planning skills.
Ability to effectively communicate and relate information to individuals of varying ages from diverse socio-economic backgrounds.
Salary Range: $64,650.00 - $103,230.00
