Responsibilities:

• Provide direct patient care and uphold personalized care plans
• Monitor patient behaviors and vital signs
• Collaborate with Registered Nurses to report findings and execute patient care plans
• Build meaningful relationships with patients to support their recovery

Qualifications:

• Current LPN/LVN licensure
• CPR certification
• Relevant experience in a hospital setting preferred
• Rehabilitation experience preferred
