Provides coordinates and directs the provision of home nursing care according to physicians orders based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager Directly Supervises and Evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.DEGREE OF TRAVELHome visits. Periodic office meetings. Must have reliable transportation current drivers license and agency-required liability insurance.'Average # of patients/day: 5-8Points per day/week: How are points broken down 30 points per week. Each type of visit and activity (documentation travel etc.) hold a different point value. Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points.Guaranteed Hours: No
if there is a situation where the contractor is unable to meet their financial needs due to low patient census please reach out to the AM to discuss floating the contractor to a nearby clinic to pick up hours.Types of Cases: Start of Care Wound Care Follow up Visits etc.Years of experience REQ: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialtyFirst-timers accepted: (Y/N) No
