**Must-Have:**- Prior critical care experience preferred- Some knowledge of CarePort, MCG, and/or InterQual preferred- Utilization Review experience preferred- Ability to handle at least 20 patients

**Job Description:**

- Expected to have knowledge of discharge planning and making referrals.- Will work on a Medical-Surgical or Telemetry unit, splitting the floor with a Social Worker.- Must function independently and work collaboratively with an interdisciplinary team and patients for discharge planning.- Expected to round on the unit with physicians/team.- Positive attitude, critical thinking, and good time management skills are essential.- Utilization Review is a separate department; they handle insurance approvals and arrange transportation.- Caseload typically consists of 17-25 patients; on pilot floors, 12-17 patients.- Involves discharge to home, community, or skilled nursing facilities (SNF).- Tasks include obtaining housing, preparing shelter packets for homelessness, handling hemodialysis packets, and making KCI referrals for wound vacs.- A binder is provided as a resource.- Primarily covers inpatient areas.- Scheduling follows a Monday to Friday, 8 AM to 4:30 PM shift (7.5-hour shifts with a 1-hour lunch).- Occasionally may require weekend or holiday work with manager approval; caseloads differ on weekends.- Dress code is business casual or scrubs (any color except Royal Blue or Maroon; neutral colors preferred); lab coat provided.- Department-specific orientation includes 2 days of hospital orientation for EMR training and 1-2 weeks with a preceptor.
