- Counsel residents and coordinate any necessary psyche services.
- Participate in behavior meetings, and coordinating room changes
- Keep accurate records for each resident you work with, including MDS logs, and report any changes in a resident's condition to the nursing department immediately
- Previous experience in a long term care facility required
- Bachelor's degree in Social Work or related field
