ensures that all families are treated with dignity and respect and given opportunity to reach the goal of services provided.This is a non-exempt position.Supervisory Responsibilities:

Position does not have supervision responsibility of staffEssential Functions of Position:Case Management / CoordinationResponds to inquiries to the Family Support Center as a first responderProvides case management to families through the Family Support Center including assessments, short term and extended navigation services, financial allocation support and resource assistanceAssists families in the development and implementation of Family Support PlansProvides training to family members utilizing intervention strategies in home/communityFacilitates support groups for families through the Family Support CenterRecruits and trains staff for home based servicesOversees home based services through supervision and case management of DDS casesServes as liaison between family, in home worker and DDSProvides assistance in locating community resources and referrals as appropriateAct as liaison with other service providers such as special education programs, health care providersMaintains documentation according to program and funding requirementsAttends internal and external meetings as neededOther duties as assigned by supervisorQualifications:BA/BS in social work or related fieldFive years experience in Human Services or related field preferredValid driver's license requiredUse of personal vehicle needed for transportation to clients’ homes, appointments, meetings and to perform varied work duties as neededUse of personal cellular telephone for work communicationBilingual in English and Spanish preferred but not required.Utilizes a flexible and responsible work style that meets evolving needs of the agency.Works with integrity and respects the dignity and value of all individuals.Exhibits mission through job knowledge, pride in work role, and advocacy.Promotes diversity and inclusion of all individuals.Works in a collaborative, compassionate manner with stakeholder/partners.
