Under the direction of the Clinical Director, the Social Worker is responsible for the quality of social works services provided to each youth. The Social Worker also provides individual and group support services and or ensures access to core services for each youth. The Social Worker oversees all required client record keeping and documentation and compliance with funding sources.
QUALIFICATIONS* Master’s degree in social work from an accredited state-approved graduate school, as defined by the Department of Education, in social work or social welfare, marriage, family and child counseling, child psychology, child development, and or counseling psychology or social psychology.* Completion of at least three semester units or 100 days of field practice or experience in a public or private social service agency at the master’s degree level.* At least nine semester units of coursework related to children and families, or 18 months of experience working with children and families.* A minimum of three semester units working with minority populations, or six months working with minority populations.* Strong supervisory skills and parenting educational skills* Be First Aid/CPR certified* Must obtain Department of Justice criminal record clearance and Child Abuse Index clearance* Must have a current TB clearance and a medical exam
JOB REQUIREMENTS AND RESPONSIBILITIES* Plans, coordinates, evaluates and reports on all required social work services for 12 youth per STRTP regulations.* Completes a transfer plan for each youth per STRTP regulations.* Coordinates, evaluates, and reports on the provisions and or access of core services for each youth.* Collaborate with Solano County Health and Human Services, Solano County Mental Health, and other placing County agencies prior to placement and throughout placement to ensure continuity of care.* Participates in case conferences and CFT meeting to ensure no lapse in service, continuity of trauma specific treatment and care and oversees transfer plans.* Develops or assists with the Needs and Services Plan.* In collaboration with the Head of Mental Health Services, the Social Worker will update every 30 days the Needs and Services Plan and ensure it reflects the recommendations of the CFT and the youth’s County Social Worker.* Supervision, collection and review of client data and information to perform social work functions.* Ensures each client file is updated and all required documentation is filed and placed.* Community outreach and resources development, liaison with referral sources, and community presentations.* Assist the Head of Mental Health Services and clinical staff as needed.* Prepare clinical and quarterly reports under the supervision of the Head of House.* Provide mental health services per MSK’s mental health contract as assigned or as needed.'* 'Benefit Conditions:* Waiting period may apply
