Provides care and services for children and families with multifaceted needs that may include stabilization in living environment, coordination of placements, and support and services when discharging from higher levels of care. Services provided are primarily in the community and clients living environment. Services may include comprehensive assessment and treatment planning, individual and family therapeutic interventions, close collaboration with clinical team to determine and guide service provision. Position will lead a team to navigate and meet the comprehensive needs of the child and family.
ESSENTIAL RESPONSIBILITIES
Provide regular and consistent services and supports to clients identified appropriate for community supportsConduct comprehensive assessments of children (ages 6-18) to determine mental and behavioral health needsProvide individual and family therapeutic interventions as appropriateManagement of clinical treatment including crisis assessment and support as neededWork closely with other members of the Community Care Team to insure clients integrated and holistic care needs are metService provision shall be conducted primarily in the clients living and other environments as necessary or appropriateProvide outreach and engagement when neededClosely collaborate with other care teams within Polara to coordinate and meet client needs (Inpatient Hospital, Crisis Stabilization Unit, Outpatient Services, Primary Care)Attend Child and Family Team Meetings (CFTs)Participates as a team member and attends scheduled staffings and supervisionPartner with client, family, and team to identify treatment goalsMaintains clinical record per agency policy and procedureAttends all team meetings, case reviews, staff meetings, supervisory sessions, and in-service trainingsManagement of all aspects of treatment, including referrals to additional resources when appropriatePerforms other duties as assigned
QUALIFICATIONS
21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel members scope of practice; or minimum 18 years old for select administrative positionsMust be able to provide or obtain an AZDPS Level One Fingerprint Clearance CardMust possess a valid Driver LicenseMaster's degree in Counseling, Psychology, Social Work, or a related field.Current licensure or eligibility for licensure as a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or similar license in the state of Arizona.Experience working with clients in a mental health setting is preferred.Strong knowledge of evidence-based therapeutic approaches for treating mental health disorders.Ability to establish rapport and build therapeutic alliances with diverse populations.Excellent communication and interpersonal skills.Empathy, compassion, and a non-judgmental approach to client care.Familiarity with electronic health records (EHR) systems and basic computer skills.Commitment to maintaining the highest ethical and professional standards.CORE COMPETENCIES
