QUALIFICATIONS
1. Master's of Social Work (MSW) from an accredited program required.
2. Require current Massachusetts Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Social Worker (LCSW). 3. 3-5 years of experience in dementia care preferred.
4. Prefer LICSW.
5. Knowledge of Alzheimer's and related dementias, disease management and caregiving.
6. Experience delivering supportive counseling, serious illness and/or grief counseling.
7. Strong interpersonal skills enabling effective collaboration and teamwork and ability to facilitate connections and collaboration between people.
8. Ability to facilitate psychoeducational programs, presentations, and community events (virtual).
9. Develop comprehensive bio-psychosocial assessments responsive to age appropriate and cultural needs and concerns; specific to patients and caregivers facing a diagnosis of Alzheimer's or related disorder.
10. Employ a range of clinical interventions such as care consultation, supportive counseling, education specific to symptom management including communication skills, managing challenging symptoms of dementia and safety planning.
11. Advocate on behalf of patients and families to gain access to services and resources.Act as a liaison to community service agencies and the MGH care team as needed.
12. Appropriately document all practice with patients.
13. Support the programmatic side of the Dementia Caregiver Support program by participating in community education programs and program development and improvement activities.
14. Participate in formal and informal clinical case reviews and seminars.
15. Attend meetings in the Division of Palliative Care and Geriatric Medicine (Division meetings, social work meetings, seminars and Grand Rounds).
Clinical Social Worker, Full Time 40 Hours/ Outpatient/ Hybrid/ Boston MA- MGH Main Campus Seeking LICSW or LCSW with experience- Excellent benefit package, newly increased pay rates!- Hybrid (Onsite/ Remote) possible The Dementia Care Collaborative within the Division of Palliative Care and Geriatric Medicine provides education, support and resources to patients and caregivers facing a diagnosis of Alzheimer's or a related disorder. Our services improve the quality of dementia care in primary and specialty care settings by addressing the needs of the patient and caregiver as a dyad. With this additional support as part of a comprehensive interdisciplinary team, the patient and caregiver are informed and supported throughout the continuum of illness. About this role:- The Clinical Social Worker is a member of the Dementia Caregiver Support team within the Division of Palliative Care and Geriatric Medicine.- They work effectively, efficiently and collaboratively as an employee of the Division of Palliative Care and Geriatric Medicine adhering to professional social work standards expected at MGH, meeting the clinical and administrative requirements.- They provide clinical services to patients/families that address psychosocial, environment, age specific and cultural issues to maximize emotional, social and physical well-being and the effective use of healthcare and community resources; conducts bio-psychosocial assessments, provides intervention and treatment as indicated.- The Social Worker is expected to be highly organized, work independently, receive minimal supervision and guidance and is expected to identify opportunities for improvement and seek out responsibility to enhance their ability to perform the work required.They will be HIPAA and Joint Commission compliant, for the Service's code or conduct and be guided by the MGH Mission, Credo and Boundaries.
