About the Job:You will provide clinical social work interventions with patients, (neonates, pediatric, adolescents, adults, and geriatrics) and families that address personal and environmental issues to maximize emotional, social and physical wellbeing and the appropriate and efficient use of health care services across the continuum. You will provide a systematic approach to evaluation of patient care. Coordinate patient care through the continuum thereby facilitating the achievement of optimal quality outcomes in relation to clinical care and cost effectiveness.
What You’ll Do:Collaborates and participates with the primary nurse and the multidisciplinary team in the ongoing clinical social work assessment of patient’s status/care managementAssist in the development, implementation, and evaluation of integrated clinical pathwaysMonitors patient’s progress to ensure care is appropriate and timelyDevelops discharge plan in collaboration with the interdisciplinary teamCoordinate’s referralsUtilization reviewWho You Are:Must demonstrate proficiency in assessing treatment and responses and adapting care to meet the needs of the patient population served; neonatal, pediatric, adolescent, adult, and geriatricMaster of Science in Social Work required; NH LICSW preferredOne (1) to three (3) years of experience in a health care system, preferably in a hospital settingExcellent clinical documentation, and successful usage of EMR systems preferredWhy You’ll Love Us:Health, dental, prescription, and vision coverage for full-time & part-time employeesShort term, long term disability, Accident insurance, & life insuranceTuition ReimbursementReferral bonusesAccrued earned time for full-time & part-time employees403b Retirement plans, with generous employer contributionsAnd more!$10,000 sign on bonus eligib#CRM
