Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Work ShiftsROTATIONAL SCHEDULE Job DescriptionFUNCTIONAL DUTIES AND RESPONSIBILITIESAssesses and triages patient coming into the clinic.Develops and maintains a system of evaluation of nursing care, practices and procedures.Presents and explains services offered by the clinic.Delegates/makes assignment based on the knowledge and competency of the assigned staff.Investigates reports, interprets and records patient responses and collaborates with appropriate members of the treatment team.Acts as patient advocate by explaining patient's Bill of Rights, Advance Directives, etc.Monitors the sign in and out computerized sheets and checks for absent and late employees.Monitors the following to ensure continuity of care.Medication Administration RecordDoctor's Order SheetPatient Education FormOther relevant interdisciplinary documentsWorks collaboratively with the ADN in planning the Staff Monthly Schedule and Vacation Schedule.Enforces staff to observe:Safety measures.Cleanliness and orderliness of the unit.Care and economy of supplies and equipment.Patient's privacy.Makes work requests for unit needs in collaboration with the Area Manager/Coordinator.Participates in unit conferences and PI process.Conducts regular staff meetings with nursing and other members of the Treatment Team as needed.Serves as liaison person with other clinics and outside agencies, e.g. Keener Shelter, Morrissania, Ryan White Program, etc.Screen consultation referrals and set up walk-in appointments.NON-ROUTINE DUTIES:Keeps anecdotal records of significant occurrences in the unit or behavior and performance of staff.Evaluates staff performance annually or Counsel staff.Attends committee meetings and serves as a member or chairperson as assigned.Assist with equipment and supply inventory, condemnation and evaluation.Sits in disciplinary hearings when needed.Orient new staff member in her/his unit.Minimum Qualifications1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA); and3.
