JOB TITLE: Team Leader FLSA STATUS: ExemptPROGRAM: Youth Assertive Community Treatment HOURS PER WEEK: 35REPORTING RELATIONSHIP: Director of Children's Clinical ServicesSALARY RANGE: $72,000 to $85,000The Youth ACT Team serves children/youth with Serious Emotional Disturbance (SED) throughout Westchester County, who are returning home from inpatient settings or residential services, at risk of entering such settings, or have not adequately engaged or responded to treatment in more traditional community-based services. Youth ACT team delivers intensive, highly coordinated, individualized services and skilled therapeutic interventions through an integrated, multi-disciplinary team approach to better achieve success and maintain the child in the home, school and community.SUMMARY of DUTIESThe Team Leader is a full-time staff member, who directs and supervises staff activities, leads team organizational and service planning meetings, provides clinical direction to staff regarding individual cases, conducts side by side contacts with staff and regularly conducts individual supervision meetings. The Team Leader is responsible for direct client services as a member of the professional staff and will provide clinical services to a small caseload. Direct services include person-centered treatment, services that promote recovery, staff supervision as well as overall management of the program. Indirect responsibilities include representing the program at meetings in the community. The Team Leader will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making and collaboration. When possible, the Team Leader will utilize evidence-based practices; always embracing a trauma-informed and person-centered approach.SPECIFIC DUTIES and RESPONSIBILITIES1. Responsible for overall program operations including; recruiting, hiring, training and evaluating of staff, monitoring of billing, revenues and expenditures, compliance with external accountability sources.2. Responsible for adhering to all standards and guidelines established by applicable regulatory authorities. 3. Responsible for program compliance with external accountability resources and the monitoring of case records. 4. Prepares monthly and annual reports of services, which provides a basis for continued program planning.5. Participation in meetings with community agencies and other service providers.6. Monitoring of intakes & discharges. Directing and coordinating clinical admissions.7. Provision of community based clinical and recovery-focused services which may include, but are not limited to, assessment and collaborative treatment planning, individual, family and group psychotherapy, crisis counseling, care management, and community consultation and education.8. Facilitation of daily morning team meetings.9. Monitoring of rotating on-call responsibilities and provides back-up to on-call staff.10. Modeling and teaching a person-centered approach, honoring choice, self- determination and shared decision-making.11. Directing day to day clinical operations.12. Coordinating the assessment and treatment planning process.13. Execute quality assurance activities.14. Develop workflows and policies and procedures for the program.15. Educating community resources regarding the Youth ACT Team and maintaining positive working relationships with community organizations.16. Use individual client data to inform clinical decision-making and identify areas for quality improvement; use data to track client progress over time.17. Follows the established agency policies and maintains appropriate professional standards.18. Establish and maintain a culture that is recovery oriented and grounded in service excellence.19. Provide supervision, mentoring, coaching and performance reviews for direct reports.20. Other designated and related duties.REQUIRED KNOWLEDGE AND SKILLS1. Effective verbal and written communication skills.2. Must complete the CANS-NY Training, be certified to use the CANS-NY, and maintain annual certification.3. Use of Motivational interviewing, recovery and trauma-informed approaches when delivering services.4. Proficient in computers.5. Effective crisis intervention skills.6. Effective leadership and problem-solving skills7. Highly organized, detail oriented, and self-motivated.8. Familiarity with and ability to use an Electronic Health Record system.9. The Team Leader shall be an experienced mental health professional with a LCSW or a Licensed Psychologist.1. The Team Leader must have at least 2 years' experience in supervisory level positions.2. The Team Leader must demonstrate positive leadership, a deep belief in recovery and self-determination, flexibility, patience, and effective written and verbal communication skills.3. It is expected that the Team Leader will be able to perform their duties with minimal supervision and consistently use good judgment.4. Computer skills in Microsoft office, spreadsheets and database management are required.5. Must be reliable and able to work independently and understand the importance of maintaining confidentiality.6. The Team Leader must have a valid Driver's License and a driving record that is satisfactory to our insurance carrier.7. The Team Leader must maintain a current New York State Defensive Driving certification.8. The Team Leader must be cleared by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State Medicaid Exclusion List.9. The Team Leader must be cleared by the NYS Office of Children and Family Services (OCFS) for instances of child abuse and/or neglect.The Team Leader must be legally eligible to work in the United States.
