The Lead Care Manager (LCM) serves as the client’s primary point of contact and works with all their providers such as doctors, specialists, pharmacists, social services providers, and others to make sure everyone is in agreement about the client’s needs and care. The LCM manages client cases, coordinates health care benefits, provides education, and facilitates member access to care in a timely and cost-effective manner. The LCM collaborates and communicates with the client’s caregivers/family support persons, other providers, and others in the Care Team in order to promote wellness, recovery, independence, resilience, and member empowerment while ensuring access to appropriate services and maximizing member benefit. This is a remote position.Working hours are 8:30am-5:00pm PACIFIC TIME.
What you’ll doRemote care duties performed through HIPAA-compliant hardware and software:
Assess member needs in physical and mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community-based referrals.Oversee client care plans and goal setting. Provide services where members reside or seek care, including office-based, telehealth, or field-based services.Connect clients to necessary social services and supports, advocating with healthcare professionals as needed. Utilize evidence-based practices like Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.Conduct outreach to facilitate ECM program linkage and log activity in the CRM system. Evaluate client progress and update SMART goals. Provide mental health promotion and arrange transportation.Complete all documentation and outcome measures within care plan timeframes. Maintain up-to-date patient records in the EMR and other business systems. Ensure program compliance through monthly reporting. Attend assigned training sessions.Your experience & qualifications
Willing and able to work Monday-Friday 8:30am-5:00pm in Pacific Time Zone, with the possibility of afternoons, evenings, and/or Saturday mornings2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populationsSuccess in a metrics-driven, feedback-oriented environmentWorking knowledge of government and community resources related to social determinants of healthExcellent oral and written communication skillsPositive interpersonal skills requiredExperience with video chatting, Google Suite, Slack, electronic health records, and comfort learning new technology is importantBilingual (Spanish/ English) a plus/preferredCompensation & benefits$24- $26 hour: The actual hourly rate paid for this position will be based on several factors, including, but not limited to: your prior experience and skills related to the position, geographic location, company needs, and current market demands.Great humans deserve great benefits!
