Program Coordinator I

for our Renascent Place site in San Jose, CA.

( *NOTE this is a Full time position with the following shift schedule: Tuesday through Friday 12pm-8pm, Saturdays 8am-4pm )

About The Role : The Program Coordinator will supervise the provision of services to program participants in the County, through direct staff supervision of staff.

General Estimation of time allocation

35 percent client care time / groups25 percent supervision of staff40 percent program support, training, coordination meetings etc.Provide supervision to non-clinical staff (peer specialist, resident services coordinators), provide evening and weekend program leadership, write reports, analyze data, develop programming.Supervise and approve Resident services monthly programming workshop calendars, newsletters, flyers etc.Role model professional behavior, crisis intervention, clinical case management, and ethical service delivery, provide training to team members as needed.Fully support assigned case load, complete all participant paperwork, clinical case management, assessments, individualized treatment planning, referrals, and other needsEnsure timely completion of MyEvolv/HMIS case notes, participant case plans, HMIS annual updates, and quarterly/yearly income assessments completed for case load and supervisee case loadsSupport participants with accessing benefits including Medi-Cal, GA, SSI, and all other eligible linkages.Provide linkage and referral to resources needed to maintain housing.Work with data team to ensure HMIS data is timely, accurate and completeProvide linkages to appropriate community services such as crisis intervention, rehab, income support and benefits acquisition, employment assistance, primary physical and mental health, substance recovery, and other servicesFacilitate psycho-educational, support groups, and group activity opportunities.Complete case notes, maintain client care time/ productivity and maintain files on services provided.Work closely with property management to resolve housing issues and help residents retain their housing.Assist Program Manager by working closely with property management and resolving housing issues with clients.Partner and collaborate with on-site stakeholders for seamless coordination.Develop relationships with agencies in the community providing resources to residents.Attend clinical, administrative, and case conferencing meetings as assignedAssist the Program Manager as needed, work as interim Program Manager when Program Manager is on leave, PTO etc.Other duties as assigned.

How You Meet Qualifications:

Professional experience in the human services, social work, psychology or related field and demonstrated experience in rental housing programs for low income, special needs individual and families. BA /BS or adequate relevant experience required.At least 1 year of experience supervising others and coordinating program activities.Must possess 2 years of field experience working with people with serious mental illness, individuals with co-occurring disorders and complex health issues and/or the chronically homeless population.Exhibits understanding of the following evidence-based practices: Housing First, Motivational Interviewing, Harm Reduction, and Psychosocial Rehabilitation.Strong engagement skills. A willingness and ability to assertively create relationships with difficult to engage residents.Sensitivity to and understanding of the special needs of the homeless.Proven ability to work independently.Excellent verbal skills. Strong organizational, detail orientated, and time management skills.Possess an understanding of and practice cultural sensitivity through open dialogue and self-exploration with diverse groups, while providing direct services.Ability to effectively intervene in crisis situations, with de-escalation techniques.Must demonstrate strong collaboration and diplomacy skills and proven track record working through challenges with community partners.Proficient in Microsoft Office computer programs.Reliable transportation and proof of a valid and current California Driver's License and current insurance along with a clean DMV record required.Ability to work flexible hours, including some weekends and evenings.Use of personal vehicle and proof of valid California Driver's License and current auto insurance, along with a clean DMV record, is required.Driving and transportation of participants when required.Flexible schedule to work evening and weekend hours as needed.
