Job Title: Outreach Coordinator-Cancer Services

Department: Cancer Services Program

Reports To: Cancer Services Manager

FLSA Classification: Non-Exempt

Job Summary: The primary purpose of this job is to work with all partnership collaborators, throughout the entire service area, to develop public education, awareness, and active outreach and in-reach plans to recruit members of the priority populations for screening in the program.

Job Results & Essential Function

Responsible for the planning, executing and evaluation of all outreach activities outlined within the NYSDOH CSP approved work plan.

Educate, motivate and encourage members of the priority populations about the importance of screening, services available, and address potential barriers to screening. In addition, encourage clients to complete all screening exams for which they are eligible. When necessary, provide education to the community on the NY State of Health.

Partner with area NYS State of Health Navigators on events and educational opportunities.

Work with partnership health providers on in-reach strategies to promote screening and prevention education. In addition, Work with providers for referral and enrollment opportunities in the NY State of Health.

Cross trains for all other program positions to assist and support all grant activities. Determines eligibility and enrolls clients in NYSDOH grants when necessary. Provides assistance and referral for enrollment NY State of Health.

Works closely with Data Manager and NY State of Health Navigators and like positions for NY State of Health referral and enrollment data.

Works with Program Coordinator to prepare outreach strategies and media/marketing plan. Works with area media contacts to ensure coverage in areas designated by the NYSDOH grant initiatives. Maintains program website and any social media activities. Secures media/marketing approval through NYSDOH, as directed.

Catalogs and maintains program marketing/media resources and contacts.

Coordinates public relations efforts to promote the program's activities and gain the support of the community, businesses, and places of work, and local legislature.

Schedules and presents educational sessions on grant initiatives; including but not limited to community, religious, and service organizations, town and village boards, and other health-related organizations.

Develops and maintains a good working rapport with the community partners, CSP state staff, and community members and agencies. Participates in area outreach events.

Represents the program and participates in meetings with local, county, and regional agencies and organizations.

Attendance at the regional and state meetings/conferences.

Meets regularly with lead agency director and supervisor. Provides progress reports on efforts.

Assists in the preparation of required reports for the NYSDOH; monthly outreach reports, quarterly reports, and others as needed.

Performs Medicaid Treatment Act applications and submission of these to NYSDOH as required.

Works with the lead agency on marketing and co-branding initiatives.

Reliable transportation. Regular travel is required throughout Steuben, Alleghany, Cattaraugus, and Chautauqua counties.

Responsible for understanding and adherence to the Standards/Code of Conduct, and the Corporate Compliance Plan at St. James Mercy Hospital.

Ensures that all educational requirements related to Corporate Compliance are implemented and adhered to as required by the CCP Education Plan.

Position Qualifications

Education & Experience

Minimum Associates Degree in Health Education or Human Services or related field and appropriate experience preferred.

Flexible hours, including some nights and weekends, as needed.

Licenses and/or Certifications

Valid Driver’s License necessary for travel

Physical Demands

Must be able to move frequently throughout the day, sits, stands, bends, and moves intermittently during working hours.

Travel in a vehicle for extended periods of time.

Language Ability

Ability to read, write and interpret documents in English such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to read, analyze and interpret common scientific/trade/technical journals. Ability to respond to common inquiries or complaints from customers, or regulatory agencies. Ability to write reports, business correspondence, and procedure manuals. Ability to speak effectively before groups of employees, patients, and vendors.

Math Ability

Basic math skills and competency in creating figures such as proportions, percentages, and/or ratios

Reasoning Ability

Responsible for monitoring own performance on assigned tasks. Self-directed: must make complex decisions independently and assist staff in complex decision making. Also, must possess high ethical standards with respect to discretion and regard for confidentiality.

Working Conditions

Must be willing to work beyond normal working hours when necessary, and ability to handle frequent interruptions.
