Position Summary:

Reporting to the Chief Nursing Officer, the Manager will lead and develop programming for HIV, HCV, STI and Substance Abuse. The Manager will report directly to CNO (or Director of PH) and receive guidance from the Medical Director for the services. The Program Manager is the "key contact" for the services and will ensure the programs provide the highest quality and evidence based care. The role requires innovative thinking and creativity to transform and coordinate services to achieve best outcomes.

Primary Responsibilities:

Program Design and Development and ImplementationCreative organization of program and processes.Ensures compliance of grant contracts (public and private), including completion and submission of reports and meeting attendance.Lead in the development grant applications.Manages data collection, correlation and analysis.Clinical QI management.Manages program evaluation, including monthly and annual reports.Prepares annual budgets, work plans and strategic plans for programs.Coordinate standardized guidelines and policies in collaboration with all service areas across CSHC.Performs direct services as needed, including counseling, screening, outreach and group sessions as needed.Development of organizational STI prevention and treatment program.Supervision:

Hire, train and evaluate team members to ensure employee and client satisfaction.Oversee all duties of team.Coordinate and facilitate monthly staff meetingsAlign Program work with Organizational Goals and Transformation into a Patient Centered Medical Home (PCMH):

Collaborate with PCMH Project Manager and PCMH team.Ensure program and team is focused on the elements of PCMH.Collaborate with all complex case managers and provide guidance for patients with HIV, HCV, STI and Substance AbuseOutreach:Seeks opportunities in the community to expand awareness around services available and support for individuals with HIV, HCV, STI and Substance AbuseOversees community programs related to HIV, HCV, STI and Substance Abuse.Assists in the development of programs to reach underserved populations and promote access to healthcare.Outreach:

Qualifications:

Bachelor's Degree required in Health & Human services or nursing program. (Masters Preferred)Experience with HIV and Substance Abuse programs preferred.Experience with grant application and management a plus.Experience in managing, mentoring and supervising preferred.Experience with high risk clients in MSM, IDU and Haitian American populations required.Strong interpersonal skills, including public communication and collaboration.Physical Requirements :

• Must be able to sit or stand for prolonged periods of time.

• Must be able to lift 10+ pounds and load onto shelves.
